Pursuant to AS 25.30.380, each party in a child custody proceeding, in its first pleading or in an attached affidavit, shall give information, if reasonably ascertainable, under oath as to the child's present address or whereabouts, the places where the child has lived during the last five years, and the names and present addresses of the persons with whom the child has lived during that period. The Child Custody Jurisdiction affidavit is form number DR-150. Sign the form, if you do not have access to a notary attach Self-Certification form (TF-835) to the affidavit. Civil Rule 11. Pursuant to Civil Rule 90.3(e)(1), each parent in a court proceeding at which child support is involved must file a statement under oath which states the parent's adjusted annual income and the components of this income as provided in subparagraph (a)(1). The child support guidelines affidavit is form number DR-305. Attach Self-Certification form TF-835 if you do not have access to a notary. Please attach proof of income, to include two paystubs and your most recent tax return. Redact all social security numbers or other sensitive information from your financial documents. Pursuant to Civil Rule 3(a) and 4(b)(2), The complaint shall be accompanied by all summonses that will be issued with the case. The court summons is form number CIV-100. Please ensure to write the case number, case caption, and your name and address on the lines provided. Pursuant to Civil Rule 90.1(f)(1)(A), A petition for dissolution or a complaint and answer for divorce, custody, legal separation, or annulment must be accompanied by an information sheet.
