A party or attorney who lodges a duplicate certifies by lodging the duplicate that it conforms to the current version of the court form. The filer's contact information must be printed on the first page of court documents. Every document must contain a case caption setting forth the correct title of the court, the city in which the court is located, the title of the action, a space for the case number, and the document name. The rule specifies the correct formatting for this information. All documents, including exhibit attachments, must be 8 x 11 inches. Documents must be two-hole punched at the top center of each page.
